Before you submit your document request:
- Check with the organization to ensure they need the form; your registration and licensure details are publicly available on our website here.
- Ensure you have selected the correct document type.
Important Information about Fees:
- Fees are in Canadian funds and include the Harmonized Sales Tax (HST).
- Fees must accompany your document request.
- Fees are non-refundable.
How To Request Proof of Current or Former Registration and Licensure
- Log in to your portal using your email and password.
- Go to the "Optional Requests – Not Required" section on the home page of the portal.
- Select the ‘Proof of Current or Former Registration and Licensure Form’.
- Submit the completed form and non-refundable fee payment.
- The Proof of Current or Former Registration and Licensure document will be sent to the organization named in your request
Other important notes:
- Document requests must be submitted by an NSCN applicant or registrant. We can not process requests from third-party organizations.
- If you are an organization that requires Proof of Current or Former Registration and Licensure, kindly direct the applicant/registrant to submit the document request.
We can provide proof of initial registration and confirmation of current licensure dating back to 1998.
